Tracy Cardin

Tracy Cardin, ACNP, SFHM is responsible for the strategic deployment and optimization of Advanced Practice Providers (APPs) at Sound Physicians. She is responsible for designing scalable clinical education, creating professional development of APPs, and providing site specific assistance with clinical integration within our practice. Tracy works closely with clinical leadership teams to drive innovation through new care models and best practices to improve performance outcomes.

Tracy brings many years of experience in APP leadership roles, most recently as the Associate Director of Clinical Integration at Adfinitas Health, where she developed scalable strategies for deployment of Advanced Practice Providers. Prior to her work at Adfinitas, Tracy was the Director of NP/PA Services of Hospital Medicine at the University of Chicago.

Tracy is a prominent thought leader in the field. She currently serves on the Board of Directors for the Society of Hospital Medicine, the first non-physician to have that honor, and contributes regularly to the Society of Hospital Medicine blog, The Hospital Leader. Tracy also serves on the National Collaborative for Improving the Clinical Learning Environment member-health disparities working group and the Association of Post Graduate APRN Programs Board of Directors.
