Sound Physicians Enters Agreement to Provide Transitional Care Services for Vibra Hospital of Western Massachusetts
Leading healthcare organization partners with long-term acute care hospital
Tacoma, Wash. – February 02, 2016 – Sound Physicians, a healthcare organization focused on improving quality and lowering cost throughout the acute episode of care, announced today an agreement to provide medical services for Vibra Hospital of Western Massachusetts. Vibra Hospital of Western Massachusetts is a 202-bed long-term acute care hospital in Springfield, Mass. Vibra Hospital belongs to Vibra Healthcare, a network of hospitals that cares for medically complex patients who benefit from long term, highly skilled care. Vibra Hospital offers pulmonary care, complex wound care, dialysis, pain management, rehabilitation and many other services.

Sound Physicians will manage a team of physicians at Vibra Hospital. Working with the hospital’s administrative team, Sound Physicians will introduce its SoundConnect workflow and communication platform to drive outcomes and foster effective communication, as it fulfills its vision of being the leader at improving quality effectively and efficiently for patients in the communities it serves.
