Finding the right fit: Considerations for a confident clinical career choice

When you’re ready to take the next step in your medical career, the right job isn’t just about compensation—it’s about finding a role that aligns with your clinical interests, work-life balance, long-term goals, and personal values. Whether you’re a newly trained physician, fresh-out-of-fellowship advanced practice provider (APP), or seasoned clinician seeking a change, here are some key factors to keep in mind before even stepping into an interview: 

Location, location, location 

Where you work matters just as much as what you do—though finding the best place for yourself or your family can feel daunting. Keep these factors in mind when evaluating your options: 

  • Regional pay differences: When researching salaries, don’t limit your comparisons by pitting two states against each other. Broaden your search to city-to-city–this reveals more meaningful differences due to cost-of-living adjustments. For example, salaries in rural areas may be higher even if the state average is lower. 
  • Maximizing earning potential: If repaying student loans or building savings is a top priority, rural locations can be a smart move. Many rural hospitals offer higher base salaries, a lower cost of living, and incentives like loan repayment programs. 
  • Compare position responsibilities: Similar job titles can come with very different responsibilities depending on the site. Always evaluate the scope of work when weighing compensation packages. 
  • Moonlighting opportunities: Some facilities offer the chance to pick up extra shifts beyond your regular schedule.  In hospital medicine, these shifts may follow a block schedule to support continuity of care, while emergency medicine often allows for more flexible, intermittent scheduling. These additional hours can be a great way to increase your income and reach financial goals faster—your recruiter can help you explore what’s available and see how much more you could earn. 

Who you serve matters 

Every hospital and facility have a unique patient population and clinical environment. Study this as closely as the job description and be sure to consider: 

  • Where you work is where you’ll live: Consider if the area matches your lifestyle. Are you looking for a vibrant city life or a quieter, family-friendly community? Do you dread a long winter, or has the cold never bothered you anyway? Ensuring you’ll enjoy your time outside of the hospital is critical to work-life balance and satisfaction. 
  • Scope of practice: The types of frequent procedures and patient cases vary widely between facilities. Talk to your recruiter to get a detailed understanding of what your day-to-day responsibilities might look like for the clinical teams you’re considering joining. 

Shift scheduling expectations 

Each clinical specialty comes with unique scheduling norms that can impact your personal and professional plans. Make sure you connect with your recruiter to clarify what “full-time” looks like before you sign: 

  • Hospital medicine (HM) and emergency medicine (EM): Full-time typically means 120-144 hours/month for EM and approximately 15 shifts/month for HM. Even this can vary by site—check with your recruiter to confirm what your contract would state. 
  • Anesthesia (AN): There is a wide variety of Anesthesiologist schedules, ranging from no-call positions to those with large call requirements and high earning capacity. The total expected hours and minimum call responsibilities may not be clearly outlined in the contract, so ensure you have a detailed conversation with your recruiter.  Zero in on what matters to you! 

At the end of the day, the right role will strike a balance between practical considerations and personal priorities. Ask yourself: 

  • Is the culture a fit? Workplace culture can have a huge impact on job satisfaction. Spend time learning about team dynamics and organizational values. 
  • Are the compensation and benefits competitive? Look beyond base salary to evaluate the full financial package, including bonuses, benefits, and loan repayment assistance. 
  • What’s the potential for work-life balance? Flexibility, time off, and autonomy is critical for long-term happiness in your role. 
  • Will I be able to grow here? Look at the leadership structure, continuing education opportunities, and potential for advancement. Is this a place you’d enjoy living and working in as you evolve throughout your life and career?
